The Chairman of the Aircraft Accident Investigation Committee of Thailand (AAICT) has formally delegated to the ATSB responsibility for the investigation into the accident on 23 September involving Qantas Boeing 747-438 registration VH-OJH.
The ATSB has now commenced an investigation into all aspects that may have influenced the circumstances of the accident.
The investigation is being conducted in accordance with the provisions of the Air Navigation Act (1920) and the ICAO Annex 13 to the Convention on International Civil Aviation. ATSB will provide AAICT with updates on the investigation and an opportunity to review the proposed final report.
The ATSB investigates for the sole purpose of enhancing safety. It is not part of the object of an investigation to determine blame or liability.
Of significant concern to the ATSB is the safety of the passenger environment during the runway overrun and the subsequent evacuation. Together with other information gathering procedures, the investigation team has developed and distributed a questionnaire to all passengers. The response from passengers has been excellent, with about 160 completed questionnaires received to date.
As with investigations into similar occurrences, other aspects to be assessed will include, but not be limited to:
- serviceability of the aircraft and its systems;
- aircraft performance;
- standard operating procedures;
- flight and cabin crew training and experience;
- flight and cabin crew performance;
- air traffic control;
- weather and airport conditions; and
- emergency services response.
An Interim Factual Report is being prepared and will be released later this week. A further Interim Factual Report may be issued prior to release of the Final Report.
